Live Well Wakefield is a social prescribing and supported self-management service.

We are a team of advisors and link workers who work one-to-one with people to help address their non-medical needs. This support could be relating to social isolation, housing, employment, education, mobility, lifestyle changes, finances, and more.

Our self-management support offers a range of short workshops and longer programmes, offering practical skills to help anyone aged 18+ to manage the impact of a long term physical or mental health condition. Topics covered include managing chronic pain and fatigue improving self-confidence by setting goals and dealing with difficult emotions. By learning and sharing skills together with others you can improve your quality of life. Our support is also open to those who are caring for someone with a long-term condition

*To register for the service you must be 18+ and live in an area covered by Wakefield Council and/or be registered with a Wakefield GP.

Live Well Wakefield Self-Management Services offers a range of courses and workshops related to self-managing long-term physical and mental health conditions. Topics covered include pain, self-confidence, long-term health conditions, and Long Covid.
