We can support individuals who meet the following criteria:

  • Currently housed in the Wakefield District, or have a local connection to the City of Wakefield or 'Five Towns' area.

  • Have an identified support need, which puts their current tenancy at risk.

  • Live in accommodation presently or are due to move into identified suitable housing.

  • Aged 16 or over.

  • Have recourse to public funds such as Universal Credit (or Housing Benefit where relevant).

SUSTAIN supports up to 175 single people and couples and 60 families across the Wakefield District at any one time.

SUSTAIN aims to complete all support with individuals within 12 months although this can be reviewing dependant on individual needs.

Support with SUSTAIN will focus on housing needs, and we will work to create a support plan with service users, which is reviewed at least every three months. We try to build resilience to difficult circumstances and focus on an individual's ability to live independently without support. We will also assist in other support areas, including employment, training and education and social networks.

Our person-centred approach means that we strive to deliver consistent support from a named support worker who will ensure needs and risk assessments, support plan, and move-on plans accurately reflect service users wishes.Please use this referral form

SUSTAIN is Wakefield Council's Housing Sustainment Pathway, and we provide housing-related support to people at risk of homelessness to enable them to keep and maintain their housing.
